My certificate was issued with incorrect name or date of birth information. Can I get a replacement?

If the certificate showing incorrect name or date of birth information was issued within the last 18 months, please speak to the Examinations Officer at the school or centre where you took your exams. They will be able to provide you with further advice and guidance.

If the certificate showing incorrect name or date of birth information was issued more than 18 months ago, we cannot send you a replacement certificate. You will need to apply for a Certifying Statement instead.

When you complete the 'Additional notes' section of the application form, please specify that you are requesting an amendment to information on your existing certificate. You must also provide copies of suitable identification document(s) which show your name and/or Date of Birth at the time you took your exams.

We consider all requests on a case-by-case basis and we cannot guarantee the outcome as these are reviewed by the results team.
