Where do we find the coversheet to sign the declaration of originality?

At the time of submission, the candidate must sign a statement confirming that they are submitting their own work. Centres countersign it to confirm that they believe the work is the candidates’ own. Centres should use the cover sheet on the samples database for this purpose, and it must appear on or before the title page of the document.
